We are working with a rapidly growing GP Federation covering 50+ practice in Kent, covering nearly 500,000 patients. The federation is divided into 9 Primary Care Networks (PCNs), each with its own leadership structure.
About the role
The role can either work as a combined full time remote assessments & home visiting or part time working in either home visiting or remote assessments. The role can either work as a locum or fixed term contract.
Working for the federation and in collaboration with the PCNs, the federation are developing a COVID Virtual Ward, including virtual assessment of COVID positive patients, a home visiting service and an assessment hub to provide assessment and treatment to housebound patients. The role of the Physician Associate will be to assess COVID positive patients virtually and undertake home visits to complete assessments and establish treatment plans in consultation with the supervising General Practitioner.
You will report to the line manager at the federation and the GP supervisor. You will develop good working relationships with GPs across the PCN, other healthcare professionals and administrative colleagues and the Lead Physician Associate.
In accordance with the Home Visiting processes, you will undertake clinical sessions alongside one of two COVID Virtual Wards (Hubs), dependent on patient demand and the needs of the Service. You will be aligned to a Hub to enable supervision and advice from a General Practitioner. In addition to home-based assessments, you will work alongside Primary Care staff in the Hub to gain further experience and supervision.
Under supervision of a GP, you will be required to:
- Undertake home visits for COVID positive patients with acute problems and chronic health conditions that require planned holistic assessment
- Work clinical sessions in the Home Visiting Service – potentially remotely, providing telephone assessments and face to face appointments, in accordance with the needs of individual patients within the service.
- Assess a range of patients whose cases vary in complexity, undertaking focused history taking and clinical assessment, advising on treatment based on local protocols and in consultation with General Practitioners.
- Provide specialist advice and treatment to COVID positive patients as delegated and agreed by the supervising GP in line with service policy and using established clinical guidelines.
- Formulate and adjust as necessary, diagnoses and treatment plans, in consultation with supervising GP where necessary.
- Consult with and refer patients to GPs, medical specialists and other health professionals as considered necessary.
- Provide medical support as required to patients referred by other healthcare colleagues.
- Organise investigations, treatment, and referral where appropriate, in line with agreed parameters, local policy and guidelines.
- Recommend and explain diagnostic tests and treatment to patients.
- Discuss and suggest appropriate choices of medications in line with competencies, local and national guidelines.
- Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes.
- Record clear and contemporaneous IT based consultation notes to agreed standards.
- Collect data for audit purposes.
- Maintain up to date knowledge of clinical guidance i.e. local guidelines, policies and procedures, NHS and NICE guidance.
To be able to work effectively in this challenging role you are going to need to bring your A game! You will be a qualified Physician Associate, registered with the PAMVR. It is essential that you have experienced of working in Primary, secondary or mental health. It is also essential that you have experience in telephone / remote consultations. Obviously you will be a team player, but it is essential that you have strong communication skills particularly in communicating sometimes complex information to patients and colleagues. You may need to be persuasive and good at negotiating to be effective in this role combined with the ability to prioritise and reprioritise and able to exercise sound judgment when faced with conflicting pressures. You will also need to be able to work under your own initiative to tight and often changing timescales.

Qualified with MSc or PG Dip in Physician Associate studies from a UK or US Physician Associate programme
Registered with the PAMVR
At least one year experience of working in primary health, secondary health or mental health sector.
